PAY DISCREPANCY FORM

If you believe you were not paid for all hours worked, please fill out this form including the days and hours that you believe were missed. Once completed, please provide this form directly to your manager.

PAID SICK LEAVE REQUEST

Please complete this online form to request paid sick leave. Your request will be sent to Human Resources and you will receive notice of approval or denial within 2 working days.

TWO-WEEK NOTICE FORM

If you plan on leaving the company, please fill out this form and provide it directly to your manager. Although we do not enjoy seeing you go, we request notice of at least two weeks before your departure.

REFER A FRIEND

Complete this form with your information and have your friend present it while onboarding. You and your friend will need to meet the requirements listed on the form to receive the referral bonus.

EMPLOYMENT VERIFICATION REQUEST

If you need verification of employment for any outside company please complete this form and return to Human Resources in person or email to EG-HR@ElitchGardens.com. Verification requests are processed within 3-5 business days.
